24-hour and Non-Occupational Coverage

• Benefit period options: 3, 6, 12 or 24month
• Income replacement up to 50% of salary
• Benefit Amounts – $300 to $3,000

Guaranteed Issue Available

• Based on 20% participation
• $750 to $2,500monthly benefit based on risk class

Pre-existing Condition Benefit

• Pays 50% of employee's monthly benefit for up to 6 weeks for a pre-existing condition, including pregnancy

Partial Disability Benefit

• Pays 50% of employee's monthly benefit for up to 3months for a partial disability (working no more than 4 hours per day)

Portable Coverage

• Employees can continue coverage when they leave employment

Waiver of Premium

• Premium payments are waived after 90 days of total disability

Optional Critical Illness Rider

• Pays double monthly benefit for up to 3months if disability is due to covered critical illness: Internal Cancer, Heart Attack, Stroke, Major Organ Transplant and Renal Failure
